Client Information
Enter names exactly as they appear on a driver's license, ID card, or passport.
Family Information
List all living children (exclude grandchildren). Add a block for each child.
Note whether the deceased child was survived by their own children.
Guardianship of Minor Children
Who would you want to raise any child under 18 if both spouses pass away? Complete only if applicable.
Successor Trustees & Executors
You serve as the initial trustee; if one spouse passes, the survivor becomes sole trustee. The choices below act only if neither of you can. Please list at least three, in order of preference. Named trustees have no authority while you are able to manage your own affairs.
Often the same person as your successor trustee, but you may name someone different.
Beneficiaries
The people you wish to receive your estate. Either give each a percentage (totaling 100%) or check equal shares. You can amend these any time.
Any specific concerns — including intended disinheritance, alternative beneficiaries, etc.
Specific items (jewelry, vehicles, collectibles, heirlooms, firearms, etc.) for a particular person.
Power of Attorney Choices
Two powers of attorney are typically used: a Durable Power of Attorney for assets & business affairs, and an Advance Health-Care Directive for medical decisions. Both are "springing" — they take effect only if you can't manage your own affairs. Your spouse is automatically your first choice; every other agent must be at least 18. These apply only if your spouse is unavailable.

These selections summarize the instructions your health-care agent would follow. Complete one set for each client.
Schedule of Assets
Do NOT enter dollar values or full account numbers. Identify each asset clearly — e.g., bank name and the last 4 digits, not the balance. This helps your plan be funded correctly.
Do not write passwords here. Note crypto/wallets (platform/type), significant online or business accounts, and where credentials are stored (e.g., a password manager).
